Required Components of the Clinical Nurse Specialist in Adult Psychiatric & Mental Health Nursing Application Packet

IMPORTANT NOTICE

The following are required components of the Clinical Nurse Specialist Application Packet; it will not be considered complete if any are omitted:
- 1. A completed, signed, and dated ANCC Certification Application.
- 2. Official transcript(s) showing all course work and clinical components in the certification specialty, and the date of degree conferral. If a post master's program was completed, you must include all transcripts from the master's and post master's program.
- Adult or Child & Adolescent Psychiatric & Mental Health CNS: If your transcript does not specify psychiatric mental health nursing as a major, then the program director must write a letter stating that psychiatric mental health nursing was the major. Attach this letter to your application.
- For degrees conferred outside of the United States or its territories, please submit a course-by-course evaluation by a foreign credential evaluation service; click here for more information.
- 3. A completed and signed Form E-1Clinical Nurse Specialist Education Preparation Validation.
- 4. If your education program had less than 500 clinical hours, submit Form E-2Clinical Nurse Specialist Supervised Clinical Practice Validation. This option will not be available beginning January 1, 2006.
- 5. Check or money order payable to ANCC or complete the credit card information section on the application form. Calculate your fees using the fee schedule here.
- 6. Completed, signed and dated ANCC Certification Application Check List.
