Description of Practice

Nurse case managers actively participate with their clients to identify and facilitate options and services, providing and coordinating comprehensive care to meet patient/client health needs, with the goal of decreasing fragmentation and duplication of care, and enhancing quality, cost-effective clinical outcomes. Nursing case management is a dynamic and systematic collaborative approach to provide and coordinate health care services to a defined population. Nurse case managers continually evaluate each individual’s health plan and specific challenges and then seek to overcome obstacles that affect outcomes. A nurse case manager uses a framework that includes interaction, assessment, planning, implementation, and evaluation. Outcomes are evaluated to determine if additional actions such as reassessment or revision to a plan of care are required to meet client’s health needs. To facilitate patient outcomes, the nurse case manager may fulfill the roles of advocate, collaborator, facilitator, risk manager, educator, mentor, liaison, negotiator, consultant, coordinator, evaluator, and/or researcher.

Eligibility Criteria

  • Hold a current, active RN license within a state or territory of the United States or the professional, legally recognized equivalent in another country
  • Have practiced the equivalent of two years full time as a registered nurse
  • Have a minimum of 2,000 hours of clinical practice in case management nursing within the last three years
  • Have completed 30 hours of continuing education in case management nursing within the last three years

All requirements must be completed prior to application for the examination

Credential Awarded: RN-BC (Registered Nurse-Board Certified)
