Nurse Opinion Questionnaire

Your perspectives are important to the American Nurses Credentialing Center's Magnet Recognition Program®. In an effort to continually improve the program, we would like to obtain your responses to this questionnaire.

By clicking the box next to the response that most closely corresponds to your opinion, indicate the degree to which each of the statements below describes the work environment in which you practice. When you have completed the questionnaire, simply click the "submit" button at the bottom of the page and your responses will be returned to us electronically, automatically, and completely anonymously.

Although you are not required to identify the facility in which you work, doing so will provide us with valuable information that can be incorporated into our evaluations of current and prospective Magnet facilities. Two of the most important ways the Magnet Program Office uses this information are for receiving information regarding organizations preparing for a site visit and gaining input on issues important to nurses regarding already designated Magnet facilities.

For organizations preparing for a site visit, the comments received and attributed to an upcoming site visit are collated and sent to the appraisal team of that particular organization. The appraisers then amplify, verify, and clarify the validity of that input during the site visit.

For already designated Magnet organizations, input is reviewed on an ongoing basis for trends. When the facility is identified, the CNO is notified if a particular issue appears to be surfacing regarding their facility and asked to follow-up with the Magnet office to ensure that the applicable Forces of Magnetism are still operating. As indicated above, all input remains anonymous.

IF you are not working at a Magnet designated organization your input is reviewed on an ongoing basis for trends and comparisons to Magnet designated organizations.

If you do choose to indicate the facility where you work, rest assured that your comments are submitted anonymously and we have no way of identifying individual respondents. If you wish to ask us a question, please click here to send an e-mail message and a staff member will respond.

Thank you for your participation!
