July 2002 AJN Launches Series on Genetics

The July 2002 American Journal of Nursing launches a periodic series on genetics with an article that discusses the major issues related to genetic testing, including obtaining informed consent for genetic testing and obtaining samples. Another article focuses on conducting a family genetic pedigree.

The July issue also includes the second article in a bimonthly series on palliative care, funded in part by a grant from the Robert Wood Johnson Foundation. This article focuses on managing pain at the end of life, including a discussion on when and how to use sedation. And don’t miss ANA’s columns: "Health and Safety" on environmental and occupational health coalitions that are advocating better work environments for nurses and other health care workers; "Issues Update" on Nursing’s Agenda for the Future and how it will address the nursing shortage; and "Washington Watch" on ANA’s ongoing fight to secure an ergonomics standard for nurses and other health care workers.

ACIP Recommendations Related to Smallpox Vaccine

In June 2001, the Advisory Committee on Immunization Practices (ACIP) made recommendations for use of smallpox (vaccinia) vaccine to protect persons working with Orthopoxviruses, to prepare for a possible bioterrorism attack and respond to an attack involving smallpox. Because of the terrorist attacks in the fall of 2001, the Centers for Disease Control and Prevention (CDC) asked the ACIP to review their previous recommendations for smallpox (vaccinia) vaccination. As a result of this review, these supplemental recommendations update those for vaccination of 1) the general population and 2) persons designated to respond or care for a suspected or confirmed case of smallpox. In addition, they clarify and expand the primary strategy for control and containment of smallpox in the event of an outbreak.

National Nursing Coaltion Wins PRSA Silver Anvil Award

Nurses for a Healthier Tomorrow, a coalition of leading nursing and health care organizations, has been awarded the Public Relations Society of America's 2002 Silver Anvil Award, which recognizes the best in strategic public relations planning and implementation.

Using the tagline "Nursing. It's Real. It's Life.", the Nurses for a Healthier Tomorrow campaign includes seven print advertisements featuring real nurses, a television public service announcement, a movie theater ad and a Web site (www.nursesource.org).

More than $1 million in monetary and in-kind contributions have been raised, the nursing shortage issue has received editorial coverage nationwide, the coalition's PSA and print ads are running nationwide, and enrollments in entry-level nursing programs increased in fall 2001, ending a six-year period of decline.

Oklahoma - Nursing Workforce Task Force Approved

The establishment of a Nursing Workforce Task Force to study and identify strategies to enhance recruitment and retention of nurses and reverse the growing shortage of nurses in Oklahoma has been approved by Gov. Frank Keating. The governor signed HB 2162, by Rep. William R. Paulk, and Sen. Keith Leftwich, both D-Oklahoma City, on May 30th, 2002.

Tom Quinn Elected 2002-2003 NSNA President

Tom Quinn, a senior student at Ocean County College, New Jersey, was elected President at the NSNA 50th Annual Convention and Alumni Reunion in Philadelphia. Mr. Quinn will begin the RN to BSN program at MCP Hahnemann University, Philadelphia, PA, next fall.

JCAHO Standards and Survey Procedures Update

Multiple decisions affecting health care organizations were made at the June meeting of the JCAHO Standards and Survey Procedures (SSP) Committee, held June 11. These decisions are pending approval by the JCAHO Board of Commissioners. All changes are effective January 1, 2003, unless otherwise noted.

Assisted Living Facilities:

Two new standards in the "Consumer Protection and Rights and Assisted Living Community Ethics" and "Leadership" chapters of the Accreditation Manual for Assisted Living (AMAL) regarding the management of medical emergencies in the assisted living facility. Revisions to standard HR.3 regarding training in basic life support were also approved by SSP.

Revisions to standards in the "Consumer Protection and Rights and Assisted Living Community Ethics" of the AMAL regarding personal rights of residents to make lifestyle choices. Field review was approved for standard revisions in the "Managing the Environment" chapter regarding the facility's smoking policy and compliance with the Life Safety Code®.

Behavioral Health Care

Revisions to the behavior management and treatment standards in the "Care" chapter of the Comprehensive Accreditation Manual for Behavioral Health Care (CAMBHC). Revisions were made to clarify the applicability of the standards, more clearly define behavior management terms, and emphasize staff qualifications, training, and knowledge regarding behavior management and support interventions, among other changes.

Health Care Networks

Revisions to the credentialing standards in the "Management of Human Resources chapter" in the Comprehensive Accreditation Manual for Health Care Networks (CAMHCN) and the addition of two new standards were approved by SSP for field review. These revisions are proposed to support safety and quality of care while, at the same time, relieving health plans and preferred provider organizations of unnecessary administrative burdens.

Home Care

Revisions to EC.1.4 and EC.2.8.2 in the Comprehensive Accreditation Manual for Home Care (CAMHC) regarding emergency management. Included in the additions is a requirement for links to the community in planning for emergencies and a requirement for the use of hazard vulnerability analyses.

Integration of rehabilitation therapy standards into appropriate standards in the "Assessment" "Care, Treatment, and Services," and "Management of Human Resources" chapters of the CAMHC. The current rehabilitation standard subset was found to be redundant of other standards and limited in applicability to all market segments.

Hospitals

A new Medical Staff standard for the Comprehensive Accreditation Manual for Hospitals (CAMH) on credentialing and supervision of volunteer licensed independent practitioners during an emergency, allowing the chief executive officer or medical staff president or their designees to grant emergency privileges upon presentation of key identification documents. This new standard is effective immediately for hospitals.

'Shortage' Means Different Things to Different Individuals

Concern about shortages of health personnel is not a new issue and preoccupies health services policy-makers, planners, employers and managers in developing and developed countries alike. However, the term 'shortage' means different things to different individuals, depending on their perspective and interests.

In a meeting conducted by the World Health Organization (WHO) on imbalances in the health workforce on March 10-12, 2002, the issue of shortages of health personnel is explored on how it is defined globally, what effects does it have on health systems in different countries and how data can best be compiled to monitor the issue. Demand for Filipino Nurses High in International Job Market

The Philippines may be churning out nursing professionals too slowly for the international job market. "The country cannot comply with the industry demand for nurses abroad. There are simply too many job orders," Labor Secretary Patricia Sto. Tomas said.

Sto. Tomas said job orders for nurses has soared since last year after the United States opened its doors for Filipino nurses. "Our nurses are preferred by hospital abroad because they provide quality work, very reliable, and are hard workers," Sto. Tomas said.

Less Pain and Fewer Adverse Events with New Hemorrhoid Procedure According to Interim Study Results

Procedure for Prolapse and Hemorrhoids (PPH) is a less painful alternative to hemorrhoidectomy, according to interim results of a head-to-head study presented at the American Society for Colon and Rectal Surgeons (ASCRS) annual meeting. In October 2001, PPH was introduced as the first new procedure for advanced hemorrhoids in 50 years. "The interim results indicate that PPH is good news for chronic hemorrhoid sufferers because they now have an effective, less painful option," said Anthony Senagore, M.D., lead investigator for the study.

Interim results found:

  • There is no difference in operating time between PPH and hemorrhoidectomy.
  • Patients having only the PPH procedure experienced significantly less overall adverse event.
  • 18% of the PPH patients experienced no pain with their first bowel movement compared to all patients who had hemorrhoidectomy experiencing pain with their first bowel movement.

Reminder - Nursing Education Loan Repayment Program Deadline June 14th

The Nursing Education Loan Repayment Program (NELRP) offers registered nurses substantial assistance to repay educational loans in exchange for service in eligible facilities located in areas experiencing a shortage of nurses. Applications are due June 14 for $8 million in grants designated to repay educational loans of nurses who agree to work two to three years at designated health facilities. HHS Releases Final State Grant Money

Health Secretary Tommy Thompson has approved many of the state bioterrorism plans thus allowing for the release of the remaining 80% or $747 million in state grants. Twenty-four states and 2 cities received the full 80% of funding. An additional 24 states and 2 cities received most of the remaining 80%, but HHS has requested that they do further plan development. Two states, Washington, D.C., Puerto Rico and the territories have all received extensions to allow for further plan development. The remaining funds will be released upon approval of each of the remaining plans.

Low Staffing Levels Increase Chances of Needle-Stick Injuries

Needle-stick injuries in hospital nurses increase dramatically when nurse staffing levels are low, University of Pennsylvania researchers conclude in the June 2002 issue of the American Journal of Infection Control. Current research indicates that patient outcomes suffer when nurse staffing levels fall; however, this is one of the first studies linking on-the-job injuries in hospital nurses with heavier workloads.

The study, conducted in 22 US hospitals, all with reputations for excellence, found that poor working conditions and high workloads were associated with 50% to 200% increases in needle-stick injuries and near-misses. The report went on to say that hospital cost-cutting might have a double-edged effect on injuries. "Nurses potentially unfamiliar with safe use of sharps were often forced to become proficient while their workloads were increasing in other ways," said Sean Clarke, RN, PhD, associate director of the Center for Health Outcomes and Policy Research at the University of Pennsylvania School of Nursing.

NACNS Call for Abstracts: Deadline July 16, 2002

The National Association of Clinical Nurse Specialists (NACNS) is accepting abstracts for the 2003 NACNS Conference, "Many Faces, One Mission: CNS Excellence in Clinical Practice and Leadership" to be held in Pittsburgh, Pa. March 27 - 29, 2003. The conference will highlight diversity among Clinical Nurse Specialists who share the common goal of bringing evidenced-based innovations to nursing care. General and break-out sessions will showcase the clinical expertise of CNSs in complex patient/family care, in programs for groups and populations, and in leadership for unit and system based change. Satellite Broadcast - The Immunization Encounter: Critical Issues

CDC will host a broadcast on "The Immunization Encounter: Critical Issues," June 27, 2002 from 12:00 to 2:30 p.m. eastern time. This live satellite broadcast will address issues related to a routine immunization clinic encounter.

The broadcast is targeted toward immunization clinic managers, staff supervisors and staff who administer vaccines including nurses, nurse practitioners, pharmacists, physician assistants, medical assistants, DOD paraprofessionals, and nursing students.

HHS Awards $30 Million to Address Nursing Shortage

The Health Resources and Services Administration in the U.S. Department of Health and Human Services announced that it has made awards to increase the number of qualified nurses and the quality of nursing services. The awards will help to ease the shortage of nurses available to provide essential health care services across the country. Programs that will receive funding include the following:

CDC Seeking Comment on Smallpox Vaccine Policy

The Centers for Disease Control and Prevention's (CDC) Advisory Committee on Immunization Practices (ACIP) is reviewing the current recommendations on the use of Vaccinia (Smallpox) Vaccine. ACIP is seeking public comment on proposed policy options. The current recommendations were issued in June 2001, Vaccinia (smallpox) vaccine: recommendations of the Advisory Committee on Immunization Practices. Summer Institute in Nursing Informatics

Baltimore, Maryland serves as the conference city for the 12th Annual Summer Institute in Nursing Informatic from July 24-27, 2002.
