AHRQ To Launch Online Patient Safety Journal

11/27/02
AHRQ and an editorial team at the University of California, San Francisco, are creating the nation's first peer-reviewed, Web-based medical journal that will showcase patient safety lessons drawn from actual cases of medical errors. Called AHRQ WebM&M (Morbidity and Mortality Rounds on the Web), the site is designed to educate health care providers about medical errors in an engaging and blame-free environment. The site, to be officially launched in February 2003, is now open for case submission. Authors of cases chosen for posting will receive an honorarium while retaining their anonymity. Every month, five interesting cases of medical errors and patient safety problems--one each in medicine, surgery/anesthesiology, obstetrics-gynecology, pediatrics, and psychiatry--will be posted along with commentaries from distinguished experts and a forum for readers' comments.

Visit http://www.WebMM.ahrq.gov to preview the site and learn how to submit cases anonymously.

AHA Issues Smallpox Vaccination Advisory

11/20/02
The American Hospital Association (AHA) has issued an advisory to its members containing recommendations on smallpox vaccination developed in the wake of the Advisory Committee on Immunization Practices' issuance of recommendations to the Centers for Disease Control and Prevention. In a cover communication with the advisory, AHA President Dick Davidson emphasized that the October recommendations substantially expand the number of hospital workers who will be offered vaccination, going beyond only those in designated hospitals to all hospitals and their employees. ACIP has estimated that some 100 health care workers per hospital will be vaccinated, or about 500,000 total vaccinations. AHA noted that the Bush administration has yet to address the critical issue of liability for adverse events associated with the vaccination.

FDA: Urgent Care's Injectable Drugs May Lack Sterility

11/19/02
The Food and Drug Administration has announced a nationwide alert concerning all injectable drugs prepared by Urgent Care Pharmacy of Spartanburg, SC. The alert is based on the lack of assurance that its products are sterile. According to the FDA, non-sterility of injectable products can represent a serious hazard to health that could lead to life-threatening injuries and death. Inspection of Urgent Care's facility revealed the company failed to have adequate controls to ensure necessary sterility. The agency reports that on Sept. 16 Urgent Care recalled all lots of its injectable methylprednisolone acetate based on reports of four patients who developed a rare life-threatening infection of the lining of the brain and spinal cord after use of their product. One patient died. The FDA is aware that Urgent Care has distributed injectable drugs to physicians, hospitals, clinics and consumers in Connecticut, Illinois, Indiana, Kentucky, Louisiana, Massachusetts, Mississippi, New Hampshire, North Carolina, South Carolina and Virginia. ANA Expresses Condolences Over Loss of Minnesota Nurse

11/01/02
The ANA was saddened to learn that a Minnesota Nurses Association (MNA) member was among the eight people killed in the crash of Sen. Paul Wellstone's plane on Oct. 25. Airplane Capt. Richard Conry, RN, was frequently requested by Wellstone to pilot his chartered flights. Conry, who had worked for American Airlines for a number of years, became a dialysis nurse a few years ago when he left the airline. Friends recall he went into that field because he knew there was a shortage in it. Even after he went back to flying, he continued working as a dialysis nurse part-time.

Colleagues described Conry as someone who "truly cared for his patients." His wife, Johanne, is also a member of the MNA. For more information, go to www.mnnurses.org.
